Cancellation Policy

Patients who cancel or reschedule their appointment less than 24 hours before their scheduled appointment time will be charged a $25 late cancellation fee.

If it patient fails to show up for a scheduled appointment, the patient will be charged a $50 No Show Fee. Two "No Shows'* will result in the discontinuation of services. A "No Show" is defined as a missed appointment that the patient has not canceled prior to the appointment time.

Please be advised that these policies are strictly enforced and failure to pay any Cancellation and/or No Show fee will result in the account being forwarded to a collection agency.

PLEASE ADVISE US if there has been any change in your insurance information (address, phone number, type of plan, coverage, etc.) since your last visit.

If a referral is required by your insurance company, you must present it at the time of your appointment. If you do not fail to bring the necessary referral, you will be responsible for any and all charges for the office visit at the time of the appointment.

It is the responsibilty of the patient to file a claim with the insurance company for reimbursement.


YOUR COPAYMENT is due at the time of your visit.

Payment is expected as services are rendered unless other financial arrangements have been made.
